Immunization
Health Requirements for School Entrance: Florida Law requires that your
child be fully immunized before entering a Florida school for the first
time. This applies to all new students in Kindergarten through 12th grade.
You must present a Certification of Immunization when registering your
child for school. The "Florida Plan for School Health Services" requires
that kindergarten through third grade students submit documentation of a
physical examination performed within the 12 months prior to initial
school entry. A student Physical Examination (HRS-H Form 3040) must be
completed and signed by a licensed practicing physician and presented to
the school at the time of registration.
Registration
Before a child can be admitted to the program, parents must provide the
following items:
Child's Birth
Certificate
Proof that the child
has received a physical examination within the last 12 months
Immunization
(including proof of tuberculin skin test and appropriate follow-up)
Proof of residence
(FPL bill, lease, deed) The above items must be on file in the
school's
office
prior to the child being admitted to class.
